Breast sagging, a.k.a. breast ptosis, can present itself in many ways and shapes. It most commonly occurs after pregnancy, due to a loss of volume, and gives the breast a flat and deflated physical appearance. This is not true sagging, and can be treated generally by breast augmentation. The procedure will lead to volume enhancement and lift the nipple's position.

Another form of sagging results when breast heaviness stretches the skin, and causes the breast to elongate and the nipple to gradually point downward. A mastectomy can remedy this by recreating a smaller skin envelope for the breast and raising the nipple. The incisions are similar to a breast reduction, and can result in a slight, overall volume reduction, but with dramatic and pleasing results.

The third type of sagging is known as pseudoptosis. Those afflicted with this see a breast that has not possibly fallen, but appears to because of inferior nipple position on the breast mound. Pseudoptosis can be sometimes treated with a breast augmentation or a "ring" mastopexy, with or without augmentation. A "ring" mastopexy results in less scarring than a full mastopexy. There is commonly a mild asymmetry in breast volume, which is noted by the plastic surgeon who will explain what additional plastic surgery may be needed to correct this.
